Participant Agreement & Waiver

Ascent Volleyball Inc. — Participant Agreement, Assumption of Risk, Release of Liability, Medical Consent, Concussion Policy, Privacy Consent & Media Release.

1. Program Participation

This Agreement applies to all activities conducted by Ascent Volleyball Inc., including but not limited to:

  • Indoor volleyball
  • Beach volleyball
  • Private lessons
  • Small group training
  • Camps
  • Clinics
  • Open gyms
  • Team practices
  • Strength and conditioning
  • Athletic testing
  • Competition preparation
  • Scrimmages
  • Tournaments
  • Off-site events
  • Any future programming offered by Ascent Volleyball Inc.

2. Assumption of Risk

I understand that volleyball and athletic training involve inherent and unavoidable risks. These risks include but are not limited to:

  • Falls
  • Slips
  • Diving
  • Jumping and landing injuries
  • Collisions with players
  • Contact with volleyballs, poles, nets and equipment
  • Finger injuries
  • Shoulder injuries
  • Knee injuries
  • Ankle sprains
  • Muscle tears
  • Heat illness
  • Dehydration
  • Concussions
  • Broken bones
  • Permanent disability
  • Death

I understand these injuries may occur despite reasonable instruction, supervision, facility maintenance, and safety precautions.

I voluntarily accept all risks associated with participation.

3. Release of Liability

In consideration for being permitted to participate in activities offered by Ascent Volleyball Inc., I release and forever discharge Ascent Volleyball Inc., including its:

  • owners
  • directors
  • officers
  • employees
  • coaches
  • contractors
  • volunteers
  • officials
  • facility owners
  • sponsors
  • representatives

from any liability, claim, demand, action, cost, damage or expense arising from:

  • injury
  • illness
  • property damage
  • disability
  • death

including claims arising from negligence, except where prohibited by law.

This release applies to all activities on or off-site.

4. Medical Authorization

I certify that I am physically capable of participating.

I agree to immediately inform a coach if:

  • I become injured
  • I feel dizzy
  • I experience chest pain
  • I believe I may have a concussion
  • I become ill

If emergency medical care becomes necessary, I authorize Ascent Volleyball Inc. to obtain medical treatment on my behalf.

I understand that all medical costs remain my responsibility.

5. Concussion Policy

Participant safety is a priority.

Participants agree to:

  • Immediately report any hit to the head or body that could result in concussion.
  • Immediately report symptoms including headache, dizziness, nausea, confusion, blurred vision, balance problems, memory issues or unusual fatigue.
  • Immediately stop participating if concussion is suspected.

Coaches may remove a participant from activity if:

  • concussion is suspected,
  • symptoms develop,
  • behaviour appears abnormal,
  • continuing participation would be unsafe.

A participant removed for suspected concussion may not return until:

  • symptoms have resolved; and
  • clearance has been provided by a qualified healthcare professional where appropriate.

Participants are expected to follow all return-to-sport recommendations.

Failure to report concussion symptoms places both the athlete and teammates at unnecessary risk.

6. Participant Responsibilities

Participants agree to:

  • demonstrate good sportsmanship
  • respect coaches, teammates, opponents and officials
  • care for equipment and facilities
  • follow all coaching instructions
  • wear appropriate athletic clothing
  • remove jewelry when requested
  • notify coaches of injuries
  • refrain from bullying, harassment, discrimination or abusive behaviour
  • participate safely and responsibly

Failure to comply may result in removal from programming without refund.

7. Privacy Policy

Ascent Volleyball Inc. respects your privacy.

Personal information collected may include:

  • name
  • date of birth
  • contact information
  • emergency contacts
  • medical information
  • attendance records
  • payment information
  • program participation history

This information is collected solely for:

  • registration
  • communication
  • emergency response
  • program administration
  • insurance purposes
  • coaching and athlete development
  • legal compliance

Ascent Volleyball Inc. will:

  • protect personal information using reasonable safeguards;
  • only collect information necessary for operating programs;
  • not sell or rent personal information;
  • only disclose information when required by law, for emergency medical purposes, insurance requirements, or with participant consent.

Personal information is managed in accordance with the Personal Information Protection Act (British Columbia).

Participants may request access to or correction of their personal information by contacting Ascent Volleyball Inc.

8. Media Release

I grant Ascent Volleyball Inc. permission to photograph, film and record me during participation.

I authorize Ascent Volleyball Inc. to use my image, voice and likeness for promotional purposes including:

  • website
  • Instagram
  • Facebook
  • TikTok
  • YouTube
  • newsletters
  • brochures
  • print advertising
  • digital advertising
  • sponsorship proposals
  • fundraising
  • presentations
  • educational materials
  • future marketing campaigns

I understand:

  • no compensation will be provided;
  • images may be edited or cropped;
  • materials may remain in use indefinitely unless removed where reasonably practicable;
  • content shared online may be copied or shared by third parties beyond the control of Ascent Volleyball Inc.

9. Communications Consent

I consent to receiving communications regarding:

  • schedules
  • weather cancellations
  • registration
  • invoices
  • newsletters
  • club announcements
  • athlete development opportunities

Communication may occur through:

  • email
  • phone
  • text message
  • approved team communication platforms

10. Refunds and Program Changes

Participants acknowledge that:

  • schedules may change;
  • coaches may change;
  • facilities may change;
  • sessions may be cancelled due to weather, emergencies or circumstances beyond the control of Ascent Volleyball Inc.

Refunds are governed by the current Ascent Volleyball Inc. Terms & Conditions and Program Policy.

11. Governing Law

This Agreement shall be governed by the laws of the Province of British Columbia.

Any legal proceedings arising from participation shall be brought exclusively in the courts of British Columbia.

12. Severability

If any provision of this Agreement is held unenforceable, all remaining provisions shall continue in full force and effect.

13. Acknowledgement

By signing below, I acknowledge that:

  • I have read this entire Agreement.
  • I understand the risks involved.
  • I understand that I am releasing certain legal rights.
  • I understand the Concussion Policy.
  • I understand the Privacy Policy.
  • I understand the Media Release.
  • I understand participation is voluntary.
  • I agree to comply with all policies of Ascent Volleyball Inc.

Parent/Guardian Consent (Required for Participants Under 19 Years of Age)

I certify that I am the parent or legal guardian of the participant named above.

I have read and understand this Agreement. I consent to the participant's involvement in Ascent Volleyball Inc. activities and agree to be bound by the terms of this Agreement on behalf of the participant, to the fullest extent permitted by law.
